Rsum : Lapproche cognitivo-biologique des troubles de stress post-traumatique

L'objet de cet article  consiste a retrouver, la partie visible de la psychologie cognitive, et de rechercher dans la partie invisible, celle qui active les liens biologique  et cognitive dans les troubles de stress post-traumatique(TSPT).

 En partant des diffrents virages thoriques posant les repres conceptuels, facilitant ainsi, la comprhension et la description de ce trauma psychique. On va ainsi aborder lapproche psychologique cognitive, en illucidant les diffrents tapes de la reprsentation mentale du TSPT. Avec une certaine prudence, nous soulignons les liens mtaphoriques entre les nouvelles sciences de la complexit et la psychologie. Simplement, pour susciter une rflexion, purement spculative.

 A lvnement des techniques dexploration surtout la neuroimagerie fonctionnelle et la multiplicit des tudes interdisciplinaires ,ont  suscits  un regain d'intrt pour l'tude des bases neurobiologiques de lensemble des fonctions cognitive, dont les psychopathologies font partis. Cela a donn lieu dinnombrable proposition des hypothses et modles cognitive explicatifs. Dans le mme cadre on a propos un model cognitivo-biologique des troubles de stress post traumatique.

- Prevalence of PTSD and ADHD among Palestinian Children  / Abdel Aziz Mousa Thabet ,Palistine

Abstract

 Background: This study aims was to identify the prevalence of trauma, PTSD, and ADHD in children living in area of   war and conflict in Gaza Strip and West Bank and relationship between trauma, ADHD, and PTSD.

 Method: A random sample of 200 children from 15 UNRWA schools in Gaza (United Nations Refugee and Work Agencies) and 150 children from 8 schools from Bethlehem and East Jerusalem were selected.  The age of children was ranged between 6-15 years. The age of children in Gaza range between 6-13 years old (Mean =9.49, SD = 2.2) and in west bank between 9-15 years old (Mean =10.05, SD =1.6).

Children were asked to fill the following questionnaires: Gaza Traumatic Events Checklist, Impact of Event Scale, while parents and teachers filled the Structured Clinical Interview of mothers and fathers for DSM-IV diagnosis of ADHD.

Results: The mean of traumatic experiences in Gaza Strip was 5.1 (SD = 3.4) and in West Bank were 7.5 (SD = 4.7).  IES scores were significantly associated with the total number of experienced traumatic events. Eighty-seven (39.2%) of children from Gaza Strip reported post traumatic stress disorder (40 and above in IES) compared to 51 (34%) of children from West Bank.

The event that was significantly associated with IES scores was day raids of their home, nigh raids at their home, tear gas inhalation, witnessing arrest of a friend , and witnessing bombardment of other homes by airplanes and helicopters.  According to parents, 16 children (8.4%) of children from Gaza fulfilled the full criteria of ADHD combined type compared to 4 (2.7%) from West Bank. According to teachers,  Ten children (5.2%) from Gaza fulfilled the full criteria of combined ADHD  type compared to 5 (3.3%) children from West Bank.  Children with many traumatic events were rated as having ADHD by parents and teachers.  Total IES scores were significantly associated with total inattention and hyperactivity scores by parent. Intrusion was significantly associated with total inattention scores by parents, and teachers; and hyperactivity scores by parents. Avoidance was significantly associated with total scores of inattention and hyperactivity scores by teachers.

Conclusion: Palestinian children are still experiencing a variety of traumatic events which lead to psychological symptoms including PTSD and ADHD.

A need for a programme to deal with children who were diagnosed as having ADHD due to environmental stressors and trauma is needed. A public awareness campaign of the effect of trauma on children well being must be enhance targeting schools, community, and youth clubs.

 - Expressive writing therapy for victims of trauma  / Abdel Aziz Mousa Thabet, Palistine

Summary : As writing appears to have many benefits, the future of writing and workbooks needs to be explored further.  It is still unclear how effective writing may be in a more naturalistic setting. A more studies using such technique compared to other sort of therapy for traumatized children will highlight the efficacy of such intervention in areas of war and conflict.

All types of therapy mentioned before are (debriefing, verbal narrative therapy, or CBT) had similarities in dealing with cognitive functions of the children and tried to help children in overcoming their adversities and trauma after exposure to traumatic events by developing coping strategies. But,  such types  are different in number of sessions, presence of children in safe place, and experience of the therapist which could influence the outcome of such therapy.

So,  such therapy need to be developed for children in the near future (e.g. techniques, structure of sessions, training for therapists, where it could be used, etc) and how this should be best evaluated....

Abstract: Drowning and Post-traumatic Stress Disorder

Psychological consequences included by disasters are more intense that organic consequences. This article aims to define a disaster and its types, then to discuss post-traumatic stress disorder. (PTSD). It will also discuss the drowing of trans-ocean ship "El-Salam 98" and PTSD of survivals and also the factors associated with psychological sequalae for this disaster.

Tunisia is the only country in the Arab world where the principle of equality between genders bas been consecrated in legal texts. In 1956, the Code of Personal Status introduced the equality of rights, through a set of provisions such as the abolition of polygamy, the institution of legal divorce and recognition of both spouses' right to seek divorce, and the setting of a minimum age for marriage to young women. From that time on, these tights have become irreversible and have continued to adapt to social transformations which have occurred in Tunisia. - Adaptation of the WISC-111 in Sudan and Japan:  A cross-cultural study / Omar Khaleefa

Abstract: A number of cross-cultural studies have been carried out particularly comparing the processes of translation and adaptation of Wechsler Intelligence Scale for Children-Third Edition (WISC-111) in many cultures and countries in Europe and Asia. No cross-cultural studies were carried out comparing adaptation of tests in Japan with African and Arab countries. The aim of the present study is to fill this gap by comparing the adaptation process of the WISC-111 in Sudan, as a poor African country, and Japan as a rich Asian country as well as to compare the performance of Sudanese and Japanese children in subtests. In Sudan, the WISC-111 (6-16 years) was translated from English to Arabic and back translated from Arabic to English and in Japan the test was translated from English to Japanese and back translated from Japanese to English. The study showed that performance tests are identical in all countries except in Sudan and Japan. Psychologists in the two countries were highly sensitive to their environment and the results showed that the WISC-111 has generally enjoyed adequate structural equivalent in Sudan as well as Japan. The adapted test was applied to a group of 330 and 1125 children in Sudan and Japan, respectively. The most remarkable finding is that the WISC-111 has high level of reliability and validity in the two countries. In Japan, the time limit for some subtests was shortened from 120 to 90 seconds, By contrast, in Sudan was increased from 120 to 150 seconds. Additionally, the study showed that Japanese children performed better in visio-spatial tests while Sudanese performed better in verbal tests....

- Cultural Perspectives On Mental Health Practice in Arab Countries /Abdel-Sattar Ibrahim

  Abstract: Individuals in Arabian cultures, as in any other culture, are forced into different life experiences, and, therefore, deliberately and inadvertently, develop many different and unique cultural values and perspectives. When we practice therapy, these differences must be taken into account before maximum success in therapy can be achieved. This presentation concentrates on exactly this premise. Based on personal practice, anthropological studies, psychosocial research, and personal observations, themes central to four major shared social and psychological characteristics including religious orientation; communal values; sex taboos; and authority-obedience orientation have been identified. The role of such dimensions in facilitating or hindering treatment effectiveness as pertain to: identifying target behavior; selecting treatment techniques; and identifying treatment goals and prognosis criteria have been analyzed. Illustrative case studies and research findings have been cited in support of our assumptions....
